College Illinois!  Authorization Form - If you have a College Illinois! 529 Plan, you must complete and submit this form  every semester  you want Illinois State University to bill College Illinois!. View the Using  College Illinois!    page for more information.

Material Charge Reimbursement Form  – Completing and submitting this reimbursement form will reimburse your material charges required by your courses. There are many courses on campus that consume materials or require students to pay a material charge to provide students full educational benefits from taking the course. An example would be the use of clay for an art project or the rental of the Bowling Center for a PE class.  Learn more about Material Charges.

Release of Information –If you would like to give someone, other than yourself, access to your Student Financial Account, the Student Accounts Office recommends adding an Authorized User to your account. Visit the How-To Guide to  Set up an Authorized User. Filling out a Release of Information form does not add an Authorized User to your account and is not a FERPA release of information.

Student-to-Student Fee Waiver - The Student-to-Student Grant fee is a  waivable fee that supports disadvantaged students in need. It is charged during Fall and Spring. You can apply for the reduction in our office through the 15th day of the current semester. Learn more about the  Student-to-Student Grant.
